Indications: - home-based primary care - hospital at home - home visits may reduce unnecessary emergency department visits & hospitalizations & may facilitate a home death for patients [1] - prenatal & postpartum home care* - home care for home-bound status - virtual home care - see home health for Medicare part A indications * 7 prenatal nurse home visits & 2 postpartum visits decreased all-cause maternal mortality * addition of home visits for the child up to age 2 years (mean number of visits, 33), plus developmental screening & referrals effectively eliminated preventable childhood mortality (0% vs 1.6%) [2] Complications: - 3 main risk factors for medication errors in the home: - transition documentation - medication reconciliation - communication among the multidisciplinary team [4] * pharmacist should be included as a member of the care team [4]
