home blood pressure monitoring

Indications: - white-coat hypertension [2] - patient participation in managing their own blood pressure [7] Procedure: Rest at least 5 minutes before reading [6] .. or 30 minutes after exercising Wait at least 30 minutes after nicotine use Wait at least 2 hours after caffeine use 3 reading 1 minute apart & average them [6] Clinical significance: - goal: systolic blood pressure < 135 mm Hg [7] - systolic BP goal corresponds with threshold for treatment with antihypertensive agent [7] Notes: Comments from clinical trials 1) blood pressure may be underestimated [1] 2) patients more likely to reach blood pressure goals [4,5] 3) self-monitoring of blood pressure improves systolic BP & may reduce stroke risk [10] 4) self-referral to the emergency department with asymptomatic hypertension from home or pharmacy blood pressure measurement admitted to hospital 3% of time [11]
