follicle center lymphoma, grade III (follicular large cell lymphoma)

Intermediate-grade lymphoma. Classification: 1) working formulation -> follicular, predominantly large cell 2) Rappaport -> nodular, histiocytic 3) Lukes-Collins -> FCC*, large cleaved &/or non-cleaved 4) Kiel -> centroblastic lymphoma, follicular * FCC: follicular center cells Epidemiology: 1) uncommon 2) < 15% of follicular lymphomas Pathology: 1) neoplastic B-cells 2) large cells, with cleaved or non-cleaved nuclei 3) mitotic figures common 4) prone to develop into diffuse lymphoma early in their course Management: 1) R-CHOP (rituximab, cyclophosphamaide, doxorubicin, vincristine & prednisone) induction [3] 2) maintenance for 2 years with rituximab [3] 3) hematopoietic stem cell transplantation may be appropriate for relapse [3] 3) prognosis poorer than with other follicular lymphomas
