fludrocortisone (Florinef)

Tradename Florinef. Indications: 1) Addison's disease 2) replacement therapy for adrenal insufficiency 3) treatment of salt-losing forms of congenital adrenogenital syndrome 4) orthostatic hypotension Adults 0.05-0.2 mg PO QD. Infants & children: 0.05-0.1 PO QD Tabs: 0.1 mg. Adverse effects: - most common (1-10%) - hypertension, dizziness, edema, congestive heart failure, convulsions, headache, acne, rash, hypokalemic alkalosis, growth suppression, hyperglycemia, suppression of hypothalamic-pituitary-adrenal axis, peptic ulcer, bruising, muscle weakness, cataracts Drug interactions: 1) aminoglutethimide 2) anticholinesterases 3) barbiturates 4) carbamazepine 5) phenytoin 6) rifampin 7) vaccines (live virus) Laboratory: - fludrocortisone in serum/plasma - fludrocortisone in urine Mechanism of action: pure mineralocorticoid.
