sodium biphosphate (Fleet phospho soda enema)
Tradename: Fleet enema. NaH2PO4
Indications:
1) relief of constipation
2) phosphate replacement
Dosage:
1) enema
a) one bottle (4.5 oz, 118 mL) once, may repeat twice
b) 1/2 dose if child > 2 yo
2) phosphate replacement:
a) 5-10 mL (0.9 g/5 mL) PO BID/TID
b) maximum dose: 45 mL/24 hours [4]
Bottles: 4.5 oz, 118 mL containing 26 g of phosphate & 4.4 g of sodium
Pharmacokinetics:
1) onset of bowel relief is in 2-15 minutes or 1-2 hours
2) elimination: enema not absorbed
Adverse effects:
1) hyperphosphatemia resulting from excessive PO intake
- phosphate nephropathy [5,6]
2) electrolyte imbalances [4]
3) dehydration [4]
4) renal failure [4]
Mechanism of action: saline laxative
