ferrous sulfate (FeSO4, Feosol)
Indications: prevention & treatment of iron deficiency.
Contraindications: hemolytic anemia

Dosage:
- 250-325 mg PO QD {TID dosing excessive [3]}
Tabs: 250, 300, 325 mg. (325 mg = 120 mg of elemental iron)
Elixir: (Feosol) 220 mg/5 mL. 5-10 mL TID
Pharmacokinetics:
1) absorbed in the duodenum & upper jejunum
a) absorptive capacity limited
b) NO benefit to > 325 mg QD [3,4]
c) NO benefit to > 15 mg (elemental iron) QD in elderly [5]
d) gastric acidity facilitates absorption
2) bound to serum transferrin
3) excreted in the urine
4) hemoglobin values response (increase) within 2-4 weeks
Adverse effects:
1) GI irritation (dyspepsia)
2) nausea/vomiting
3) dark stool
4) constipation* (especially with doses > 325 mg QD)
5) diarrhea
* constipation much more common than diarrhea
Drug interactions:
1) FeSO4 may decrease absorption of tetracyclines
2) antacids & milk may decrease aborption of FeSO4
