ethionamide (Trecator-SC)
Tradename: Trecator-SC
Indications:
1) treatment of tuberculosis & other mycobacterial infections, including Mycobacterium avium complex
2) used in conjunction with other anti-tuberculous agents when 1st line therapy has failed or resistance has been demonstrated
Contraindications: severe hepatic impairment
Caution: use with caution in patients taking isoniazid or cycloserine
Dosage:
1) 500-1000 mg/day divided BID
2) children 15-20 mg/kg/day divided BID; max 1000 mg/day
3) take with meals
4) neurotoxic effects may be relieved by coadministration of pyridoxine 50 mg QD
Tabs: 250 mg. Dose adjustment in renal failure:
creatinine clearance dosage
10-50 mL/min 100%
< 10 mL/min 50% of dose
Pharmacokinetics:
-> elimination 1/2life 2.1 hours
Adverse effects:
1) common (> 10%)
- anorexia, nausea/vomiting
2) less common (1-10%)
- hepatitis, jaundice, peripheral neuritis, psychiatric disorder, postural hypotension, metallic taste
3) uncommon (< 1%)
- hypothyroidism, goiter, hypoglycemia, drowsiness, dizziness, optic neuritis, seizures, headache, rash, stomatitis, gynecomastia, abdominal pain, diarrhea, thrombocytopenia
4) other
- hypersensitivity [4]
Drug interactions:
1) cycloserine
2) isoniazid
3) increased toxicity with rifampin
Test interactions:
- decreased serum thyroxine
