ergotamine (Ergostat)
Tradename: Ergostat.
Indications:
- prevention & treatment of vascular headache (migraine)
- irritable bowel syndrome [4]
Contraindications:
1) peripheral vascular disease
2) hepatic disease or renal disease
3) hypertension
4) peptic ulcer disease
5) sepsis
6) avoid use during pregnancy
Dosage:
- 2 mg SL, may repeat every 30 min X 2
- Max 5 tabs/week
Tabs: 2 mg.
Pharmacokinetics:
1) absorption increases with caffeine coadministration
2) metabolized by liver
3) metabolites eliminated in the bile
Adverse effects:
1) tachycardia
2) bradycardia
3) arterial spasm
4) claudication
5) vasoconstriction
6) rebound headache may occur with sudden withdrawal
7) localized edema
8) numbness & paresthesias of fingers & toes
Drug interactions:
1) propranolol in combination may result in severe vasoconstriction
2) erythromycin & clarithromycin in combination may result in ergot toxicity
Mechanism of action:
1) direct stimulation of vascular smooth muscle
2) vasoconstriction of peripheral & cerebral vessels
3) antagonism of serotonin activity
