eosinophilic gastritis

Etiology: - idiopathic, possibly allergic - generally occurs in atopic persons (suggested by eosinophilia) Epidemiology: rare Pathology: - infiltration of eosinophils into the stomach - small intestine may be involved Special laboratory: - upper GI endoscopy - gastric mucosa may appear normal - gastric biology - eosinophils in gastric mucosa > 10/HPF - allergy testing to identify foods to eliminate Differential diagnosis: - drugs - parasitic infections - bacterial infections - inflammatory bowel disease - Crohn's disease - vasculitis - polyarteritis - allergic vasculitis - hypereosinophilic syndrome - connective tissue disease - scleroderma - myeloproliferative disorders Management: - food elimination (see eosinophilic esophagitis) - glucocorticoids vs elemental diet if food elimination fails
