cefamandole (Mandol)
Tradename: Mandol. 2nd generation cephalosporin.
Indications:
- bacterial infections due to susceptible organisms
- lower respiratory tract infection
- pneumonia
- intra-abdominal infection
- bacterial peritonitis
- skin or soft tissue infection
- infectious arthritis
- osteomyelitis [5]
- prophylaxis for perioperative infection [5]
Dosage: 0.5-1 g IV/IM every 4-8 hours.
Children: 50-100 mg/kg/day divided every 4-8 hours.
Serious infections 150 mg/kg/day.

Anaerobes
- Bacteroides melaninogenicus

Adverse effects:
1) bleeding secondary to coagulopathy (in elderly)
-> decreases vitamin K synthesis
2) disulfiram-like reaction
a) nausea/vomiting
b) headaches
c) flushing
3) allergic urticarial rash
4) eosinophilia
5) positive direct Coomb's test
6) fever
7) increased AST
8) increased creatine kinase
9) diarrhea
10) thrombophlebitis
11) thrombocytopenia
12) granulocytopenia
Drug interactions:
1) coadministration with alcohol may produce a disulfiram-like reaction
2) cefamandole may enhance the effect of warfarin
Laboratory:
1) specimen:
a) serum
b) keep specimen on ice-water
c) centrifuge at 4 degrees C
d) remove cells as soon as possible
e) freeze at -70 degrees C until assayed
