cavernous sinus syndrome
Etiology:
- brain tumors
- vascular pathology
- Tolosa-Hunt syndrome
Clinical manifestations:
- periocular pain
- paresthesias
- facial paresthesias involving cranial nerve V2 associated with tumors
- diplopia
- cranial nerve involvement
- optic nerve involvement associated with brain tumors
- Tolosa-Hunt syndrome always painful
Laboratory:
- CSF analysis useful only in unusual cases
Radiology:
- MRI generally distingushes vascular pathology from brain tumors
- MRI negative in Tolosa-Hunt syndrome
Management:
- Tolosa-Hunt syndrome responds to glucocorticoids
