Procedure: - 1 point for - age 41-60 years - minor surgery - BMI > 25 - lower extremity edema - varicose veins - recent or current pregnancy - estrogen use - recurrent spontaneous abortion - sepsis within the last month - pneumonia within the last month - severe lung disease - abnormal pulmonary function - inflammatory bowel disease - acute myocardial infarction - heart failure within last month - medical patient at bed rest - 2 points for - age 61-74 years - arthroscopic surgery - major surgery lasting > 45 minutes - malignancy - bedrest for > 72 hours - immobilizing orthopedic cast - central venous catheter - 3 points for - age >= 75 years - personal history of venous thromboembolism - family history of venous thromboembolism - thrombophilia (congenital or acquired) - heparin-induced thrombocytopenia - 5 points for - stroke or spinal cord injury within last month - elective arthroplasty - hip fracture - pelvic fracture - leg fracture
