Community Aging in Place - Advancing Better Living for Elders (CAPABLE) Program

The CAPABLE program focuses on the intersection between disability and home environment. Implementation through Medicaid waiver-funded Procedure: - entails home visits over 5 months by a team comprising a nurse, an occupational therapist, & a home repair person, each trained to support patient-identified goals Clinical significance: - CAPABLE program can reduce disability scores by 30% - reduction in disabilities, falls, hospitalizations, & long-term care spending - cost of provision of home modifications & adaptive equipment averaged $1,300 per home; there was no cost to participant - calculated cost-savings of $867 per month per participant (2021)
