burning mouth syndrome

Etiology: - disorder associations - food allergy - drug allergy - diabetes mellitus - menopause - generalized anxiety disorder - depression - lichen planus - denture use - dental treatment - postulated causes - vitamin B12 deficiency - iron deficiency Epidemiology: - prevalence: 0.7% (USA); up to 7% in Europe - more prevalent in older adults & women - female:male ratio = 7:1 Pathology: - salivary function is normal Clinical manifestations: - burning sensation or other dysesthesia of the oral mucosa - generally involves anterior 2/3 of tongue, hard palate & lips - gingival tissue less frequently involved - symptoms rarely involve the soft palate or mouth floor - dysgeusia common - xerostomia common Laboratory: - serum vitamin B12 - iron studies - serum iron, TIBC, serum ferritin - fungal culture of oral cavity for oropharyngeal candidiasis Management: - no treatment guidelines - treatments that have been used include - topical rinse - lidocaine, capsaicin, doxepin - oral agents - antidepressants - opiates - anticonvulsants - benzodiasepines
