biliary cast syndrome

Etiology: - liver transplantation for primary biliary cirrhosis Pathology: - dilatation of the lower end of the common bile duct - filling defect at the upper end of the common bile duct Laboratory: - bilirubin in serum -> marked hyperbilirubinemia - mildly elevated serum ALT, serum AST (< 100 U/L) Radiology: - magnetic resonance cholangiopancreatography: thinning of intrahepatic bile duct Management: - surgery & a 2nd liver tranplantion may be necessary
