betaxolol (Kerlone, Betoptic)

Tradenames: Kerlone, Betoptic. Indications: 1) glaucoma a) open-angle glaucoma b) secondary glaucoma c) increased intraocular pressure 2) systemic beta-blocker - hypertension Cautions: 1) bronchial asthma 2) sinus bradycardia 3) 2nd & 3rd degree AV block 4) heart failure Dosage: 1) ophthalmic: 1 drop OU BID 2) systemic: - start 10 mg PO QD - may iincrease to 20 mg/day Solution: 0.25%, 0.5%. Tabs: 10 & 20 mg. Adverse effects: 1) ocular - ocular stinging, photophobia, keratitis, tearing, erythema, itching, decreased corneal sensitivity 2) systemic effects a) not common (1-10%) - bradycardia, palpitations, edema, congestive heart failure, cold extremities, dizziness, fatigue, lethargy, headache b) uncommon (< 1%) - nervousness, depression, chest pain, thrombocytopenia, hallucinations Mechanism of action: 1) beta-1 adrenergic receptor antagonist; little effect on beta-2 adrenergic receptors 2) reduces production of aqueous humor 3) no effect on accomodation or pupillary size 4) intraocular pressure diminishes by 20-35%
