nasal polyps with asthma; aspirin-exacerbated respiratory disease; Samter syndrome

Pathology: - mucosal swelling of the paranasal sinuses & nasal membranes - formation of nasal polyps - asthma precipitated by exposure to cyclooxygenase-1 [4] Clinical manifestations: - nasal polyps - asthma & rhinosinusitus precipitated by exposure to aspirin or other NSAID [4] - aspirin sensitivity Management: - avoid aspirin, NSAIDs - stepped asthma care, including leukotriene receptor antagonist [4] Notes: - COX2 inhibitors may be better tolerated than non-selective NSAIDs - patients usually will tolerate celecoxib better than meloxicam
