Indications:
1) useful in conjunction with other tests in evaluation & management of androgen disorders
a) androgen excess
b) androgen deficiency
2) monitoring of glucocorticoid treatment of congenital adrenal hyperplasia
Specimen:
1) serum
2) place on ice & process in 1 hour
3) store frozen
4) specimen should be collected at least 1 week before or after menstrual period
Reference values:
1) male (adult): 75-205 ng/dL (2.6-7.2 nmol/L)
2) female (adult): 85-275 ng/dL (3.0-9.6 nmol/L)
3) prepurtal: 8-50 ng/dL (0.3-1.7 nmol/L)
Increases:
1) clinical disorders
a) polycystic ovary disease (some)
b) hirsuitism (some)
c) congenital adrenal hyperplasia
d) Cushing's syndrome
e) ectopic ACTH-producing tumor
f) hyperplasia of ovarian stroma
g) ovarian tumor
h) osteoporosis in females
2) pharmaceutical agents
-> in vivo effects
-> corticotropin, clomiphene, cyproterone acetate, levonorgestrel, metyrapone
Decreases:
1) clinical disorders
a) sickle cell anemia
b) adrenal failure
c) ovarian failure
2) pharmaceutical agents
-> in vivo effects
-> dexamethasone
