amiloride (Midamor, Amipramidin, Modamide)
Tradename: Midamor.
Indications:
- lithium-induced nephrogenic diabetes insipidus
- hypercalcemia
- hypokalemia
- hypertension
- edema
Dosage: 5-10 mg PO QD.
Tabs: 5 mg.
Dosage adjustment in renal failure:
- lower dose with eGFR < 30 mL/min [3]
Adverse effects:
- hyperkalemia, hyponatremia
Drug interactions:
- hyperkalemia potentiated by concomitant use of ACE inhibitors, NSAIDs, diabetes.
Mechanism of action:
1) inhibits collecting duct lumenal Na+ channel
2) blocks uptake of Li+ by the distal tubule & collecting duct
3) mild extracellular fluid volume contraction
4) increased Na+ & H2O absorption at the proximal tubule
5) decreased urine volume
