alarm features of dyspepsia

Clinical manifestations: 1) age > 55 years with new-onset symptoms 2) family history of proximal gastrointestinal cancer 3) unintentional weight loss 4) gastrointestinal bleeding 5) history of peptic ulcer 6) progressive dysphagia, odynophagia 7) unexplained iron-deficiency anemia 8) persistent vomiting 9) palpable mass or lymphadenopathy 10) gastric surgery 11) jaundice (not mentioned in [1]) 12) diaphoresis, unexplained fatigue, nausea, vomiting, exertional symptoms suggest angina pectoris masquerading as dyspepsia Special laboratory: - upper gastrointestinal endoscopy - electrocardiogram if any doubt this could be angina pectoris* * atypical presentations of cardiac ischemia & myocardial infarction are more common in women, older adults, & patients with diabetes mellitus
