acetohexamide (Dymelor)

Tradename: Dymelor. Dosage: start with 250 mg PO QD, max 1500 mg/day divided BID Tabs: 250 & 500 mg. Active metabolite: hydroxyhexamide. Adverse effects: 1) transient leukopenia 2) thrombocytopenia 3) hemolytic anemia 4) pancytopenia 5) gastrointestinal disturbances 6) headache 7) tinnitus 8) photosensitivity 9) allergic skin reactions 10) intolerance to alcohol 11) hypoglycemia 12) hepatic dysfunction [2] Drug interactions: -> agents that increase risk of hypoglycemia -> sulfonamides, beta-blockers, salicylates, clofibrate, phenylbutazone, probenecid, dicumorol, warfarin, chloramphenicol, MAO inhibitors, alcohol [2] Laboratory: 1) specimen: serum 2) methods: HPLC, GLC, color 3) interferences: a) color: hemolysis, primary & secondary amines b) GLC: plasma lipids Mechanism of action: 1) sulfonylurea 2) small diuretic effect
