The 4Ms - what Matters - Medication - Mentation - Mobility Clinical significance: - the 4Ms identify core issues that should guide all decision making in the care of older adults [1] - the 4Ms are relevant regardless of - disease &/or comorbidities - functional problems - cultural, racial, ethnic, or religious background Management: - two key drivers of age-friendly care: - assess the 4Ms for each older adult in your care - act on incorporating the 4Ms into the plan of care - use the Plan-Do-Study-Act cycle to integrating the 4Ms into clinical care [1] - a team-based approach is most likely to succeed
